I chose Family Medicine and still support its ideals.   The full range of Family Medicine includes obstetrics, minor surgery, hospital care, long term care, management of chronic problems and acute illness, and perhaps most importantly - preventive care.  I had the opportunity to do that for 15 years in great small town practice, and even the opportunity to develop both a Rural Residency Program and help implement a full EMR within that practice.

Moving into Emergency Medicine allowed me to redirect my career toward Informatics.  This was a difficult decision as Family Medicine is one of the most cost effective disciplines, and  ER is one of the most expensive.   The goal of Family Medicine is to keep patients out of the ER, and the goal of ER is to get them into follow up with their Primary Care Provider.  But the overwhelming need for both of these disciplines is information management.  Information technology can improve operations and patient outcomes by allowing these fields to work together more effectively.
